Form Maker Plus — a free form maker and CSV-to-PDF mail-merge tool
Form Maker Plus is a completely free form maker with bulk export, running with no backend. Build a form by dragging fields, import a CSV, and generate one styled PDF per row — bulk certificates, invoices, badges or letters — all in your browser. No install, no add-on, no account, no sign-up. Your CSV is parsed locally and never uploads.
What the terms mean
- Mail merge
- Taking one document template plus a table of data, and producing one finished document per row of that table. Word, Google Docs add-ons and document-automation services all do this on their own servers; Form Maker Plus does it in the browser tab.
- One row → one PDF
- Each row of your CSV becomes its own styled PDF. Twenty names in a spreadsheet produce twenty certificates. The column headers are the merge fields.
- CSV
- A plain-text spreadsheet: one header row naming the fields, then one line per record. Every spreadsheet app exports it, which is why it is the input format here.
- No backend
- The CSV is parsed and every PDF is built by vendored jsPDF inside your browser. There is no server, no upload, no account and no add-on to install.
How it works
- Drag field types onto the blank A4 form and edit each one — the same eight types as Form Maker, up to fifteen fields per form.
- Fill the form in for one record and press Add to Excel/CSV. That record becomes a row in the Records table underneath, with your field labels as its column headers.
- Press Download CSV at any point to take that table away as a spreadsheet.
- Press Export 15 PDFs Zip and every row comes back as its own styled PDF — up to fifteen rows in a pass — built by vendored jsPDF in the browser tab and downloaded together as one zip.
