Form Maker — a free form maker and PDF form builder

Form Maker is a completely free, private, no-account form maker for building a form. Drag fields onto the page, fill them in, and export a clean, styled, fillable A4 PDF — no account, no sign-up, and no upload. Unlike the online survey builders, your form is a self-contained PDF you own, not a page hosted on someone else's server. The PDF is built right in your browser via vendored jsPDF.

How it works

  1. Drag a field type from the palette onto the blank A4 form. Eight types are available — text, text area, number, email, date, checkbox, dropdown and URL — up to fifteen fields per form.
  2. Click the pencil on any field to set its label, placeholder, character limit, number range, required flag and dropdown options. Drag a field to move it up or down the page.
  3. Fill in the form header: a brand name, an event title, and a square logo under 200 KB if you want one.
  4. Press Export to PDF. Vendored jsPDF builds the styled A4 document inside the browser tab and it downloads straight away.
